Snow Clearing 2020-2021 - Seasonal Help Package
This seasonal help package is for the purpose of our “on call list” for snow clearing. This is not a permanent position and is on a first come, first serve basis.
We have compiled a list of forms that must be filled out and returned to our office.
You will not be put on the “on call list” until these forms have been completed and returned to our corporate office.
These forms help us to set you up in our system, to insure that you are eligible to work in the United States and that you have a valid VA. Driver’s license/CDL medical card and a good driving record.
1. Employee Information – this form gives us general information about you and who we need to contact in case of an emergency. Please list at least 2 phone numbers to reach you, 1 for our texting system to keep you up to date.
2. Form W-4 – this form helps you to decide the number of exemptions you would like withheld from your federal taxes.
3. Form VA-4 – this form helps you decide the number of exemptions you would like withheld from your state taxes.
4. Employment Eligibility Verification (Form I-9) – this form verifies your eligibility to work in the United States. You must provide with this form at least one form of identification from List A - or – 2 forms of identification (one coming from List B and one from List C).
5. Paychex Direct Deposit Sign Up – this form is for you to indicate whether you would like for your pay to be directly deposited into your account. This form must be returned to our office with a voided check to authorize us to make deposits to your bank. (this is optional)
6. CDL Supplemental Application. This form is an indication of what transportation you have driven, and any accidents that you have been involved in.
7. Employment Application –please complete, sign and return to the office for your files.
8. In addition to these forms, we require that we have a copy of your driver’s license (VA)
9. We also require a copy of your CDL medical card.
10. Please include a copy of your driving record.

All checks will be mailed or paid by direct deposit. Checks will be mailed the week after the completion of the snow event. (Example: should snow event end on Sun – check will not go out that Friday but the week following).
